Hospice Alternative Residence Team (H.A.R.T.)
Hospice works with nursing facilities, assisted living communities, and other retirement communities to provide the full range of hospice services for their residents. All services available for hospice patients in their homes are available for terminally ill persons in long term care settings. Hospice serves as an extension of these communities' services, providing expertise in end-of-life care and grief support.
WHAT SERVICES ARE PROVIDED?
To enhance the care and daily attention provided by community staff, the Hospice Alternative Residence Team (HART) customizes a plan of care for each resident. Our services include:
- management of the resident's pain and other symptoms
- coordination of medical services with the resident's physician
- case management and coordination with outside resources
- support for family members and others in their role as caregivers
- 24 hour nursing support and consultations for emergencies on nights and weekends
- spiritual care
- grief support for those experiencing or anticipating a loss
- assistance with advance care planning
- education and support for residents, families, and staff
- in-home aides to address activities of daily living needs
WHO IS ELIGIBLE FOR OUR CARE?
- Residents referred by a physician
- Individuals desiring care focused on pain control and symptom management
- Individuals of all ages, residing in nursing facilities, skilled, long-term care and assisted living, and retirement communities in our service area
- Residents facing a terminal illness with a life expectancy of six months or less
HOW CAN YOU OBTAIN SERVICES?
To make an inquiry about our services, talk with the community staff or a physician. Our referral center staff will work with the patient's physician to initiate services.
